Reproductive Genetic Counseling Referral
Indications for referral include but are not limited to advanced maternal age, positive prenatal screening, recurrent miscarriages, teratogens/exposures, personal/family history of a genetic disease, and abnormal ultrasound findings.
Natera Telehealth genetic counseling appointments include the following: a review of the genetic parents’ family histories and available medical records, a reproductive risk assessment, and a detailed discussion of the risks, benefits, and limitations of available screening/diagnostic testing. These video appointments are conducted over Zoom; Natera will provide a Zoom link when the appointment is scheduled.

Please fax the following to 650-412-2305:
- Completed Reproductive Telehealth Genetic Counseling Referral Form
- Medical records, including test results and ultrasound reports, that support the consultation
- Copy of the patient’s insurance card (front and back) and HMO authorization, if required
Natera will confirm receipt of the referral.

Schedule
- The email confirming receipt of the referral and necessary records will contain a link to Natera’s scheduling system. Once referral and records are received, Natera will email the scheduling link to the clinic or patient to schedule the appointment.
- Natera patient form is available here. Completed form (Natera Reproductive Genetic Counseling Questionnaire and Patient Acknowledgement) must be returned to Natera (fax 650-412-2305 or secure email telehealthgc@natera.com) prior to the patient’s appointment.
If the patient does not have access to a printer and/or fax, please provide the forms to them and send to Natera prior to the visit. If you have any questions, please email telehealthgc@natera.com.
