Hereditary Cancer Genetic Counseling Referral

Indications for referral include but are not limited to personal and/or family history of cancer, positive hereditary cancer genetic test result, personal history of polyps and tumor testing suggestive of a possible hereditary cancer syndrome.

Natera Telehealth genetic counseling appointments include the following: a review of patient’s family history and available medical records, a risk assessment, and a detailed discussion of the risks, benefits, and limitations of available testing. These video appointments are conducted over Zoom; Natera will provide a Zoom link when the appointment is scheduled.

Please fax the following to 650-412-2305:

  1. Completed Hereditary Cancer Telehealth Referral Form
  2. Medical records, including test results and pathology reports that support the consultation
  3. Copy of the patient’s insurance card (front and back) and HMO authorization, if required

Natera will confirm receipt of the referral.

Schedule

  1. Upon receiving the referral and corresponding medical records, Natera will send a confirmation email to the clinic and provide a link for the appointment to be scheduled.
  2. Natera patient form is available here. Completed form (Hereditary Cancer Genetic Counseling Questionnaire) must be returned to Natera by fax (​650-412-2305) or secure email (telehealthgc@natera.com) prior to the patient’s appointment.

If the patient does not have access to a printer and/or fax, please provide the forms to them and send to Natera prior to the visit. If you have any questions, please email telehealthgc@natera.com.
