Hereditary Cancer Genetic Counseling Referral
Indications for referral include but are not limited to personal and/or family history of cancer, positive hereditary cancer genetic test result, personal history of polyps and tumor testing suggestive of a possible hereditary cancer syndrome.
Natera Telehealth genetic counseling appointments include the following: a review of patient’s family history and available medical records, a risk assessment, and a detailed discussion of the risks, benefits, and limitations of available testing. These video appointments are conducted over Zoom; Natera will provide a Zoom link when the appointment is scheduled.

Please fax the following to 650-412-2305:
- Completed Hereditary Cancer Telehealth Referral Form
- Medical records, including test results and pathology reports that support the consultation
- Copy of the patient’s insurance card (front and back) and HMO authorization, if required
Natera will confirm receipt of the referral.

Schedule
- Upon receiving the referral and corresponding medical records, Natera will send a confirmation email to the clinic and provide a link for the appointment to be scheduled.
- Natera patient form is available here. Completed form (Hereditary Cancer Genetic Counseling Questionnaire) must be returned to Natera by fax (650-412-2305) or secure email (telehealthgc@natera.com) prior to the patient’s appointment.
If the patient does not have access to a printer and/or fax, please provide the forms to them and send to Natera prior to the visit. If you have any questions, please email telehealthgc@natera.com.
